While every effort will be made to ensure the safety of participants, I understand that such risks cannot be entirely eliminated. Agree Disagree Release of Liability * In consideration of my child’s participation in swim lessons, I hereby release and hold harmless The Neighborhood Mermaid, its employees, agents, and representatives, from any and all liability, claims, or causes of action that may arise from injury, illness, or damage sustained by my child during participation in swim lessons, except where caused by gross negligence or intentional misconduct. Agree Disagree Medical Treatment Authorization * In the event of an emergency, I authorize The Neighborhood Mermaid to secure medical treatment for my child if deemed necessary. I agree to assume financial responsibility for any medical expenses incurred as a result of such treatment. Agree Disagree Photo/Video Release * I grant permission for The Neighborhood Mermaid to take photographs or videos of my child during swim lessons for promotional purposes.
